40-49 year olds Diabetes Risk Assessment 

The aim of the type 2 diabetic risk evaluation is to assess people aged 40-49 years of age  who are at high risk of developing Type 2 Diabetes .

The risk assessment tool helps us to assess the risk of you developing type 2 diabetes over the next 5 years and assist you make achievable lifestyle changes to this preventable disease.

The risk assessment can be undertaken online or within our medical practice;

The risk assessment tool is based on a points system which, when added together, gives risk and is then classified from low to high.

  • Those at moderate risk are advised to have a discussion with their GP to make beneficial lifestyle changes to aid in the prevention of this disease.  If you are between 45-49 you are eligible for a Medicare funded health assessment.

  • Those at high risk are advised to consult with us about booking in for a Medicare funded health assessment and further pathology testing.

** If diagnosed as a Type 2 Diabetic throughout this process you are then eligible for a Medicare funded Diabetic Care Plan which involves a Team approach to help you manage your condition.  Within the Care Plan you will be monitored quarterly with pathology testing etc and these appointments can also be Medicare funded if booked as a review.


Risk factors assessed include the following:

  • lifestyle risk factors, such as smoking, physical inactivity and poor nutrition;

  • biomedical risk factors, such as high blood pressure, impaired glucose metabolism and excess weight; and

  • a family history of a chronic disease.
